Resources for Child Care Providers

The expectations of people who provide child care in Canada have never been greater. These forms and checklists for child care practitioners and other caregivers will:

  • Promote safe and healthy practices in child care settings; and
  • Make reporting and planning quick and easy.

Each PDF is free to download and share with colleagues and caregivers. They all come from the latest (2015) edition of Well Beings: A Guide to Health in Child Care. 

  1. Managing Infections: This chart shows common—and uncommon—illnesses in child care, including signs and symptoms, how they spread, and when to report to public health authorities or ensure a sick child stays at home.  

  2. Specific Medical Conditions: Use these basic “action plans” to help support children with special health needs—anaphylaxis, asthma, cystic fibrosis, and type 1 diabetes. 

  3. Consent Forms and Record Sheets: Obtain pre-authorized consent from parents for giving medication or providing emergency care and transportation.

  4. Injury Report: Document any injury using this form to identify what went wrong and to how to prevent a recurrence. 

  5. Cleaning and Sanitizing Schedule: This checklist provides suggested methods and frequencies for cleaning and sanitizing within any child care environment.
